Information for Clinical Staff

Scoliosis Referral Form 2022

The current Referral Criteria as advised by the Cabinet Secretary for Health, the Chief Executives of the Scottish Health Boards and National Services Division, June 2012. We are only allowed to accept consultant-to-consultant referrals / tertiary referrals.

for young people (up to 17th birthday) with:

  • idiopathic scoliosis (infantile, juvenile and adolescent)
  • congenital spinal deformity
  • severe Scheurmann’s kyphosis
  • neurological conditions cerebral palsy and others syndromic conditions
  • Muscular Dystrophies to include Duchenne and Spinal Muscular Atrophy
  • spondylolysis / spondylolithesis
  • Neurofibromatosis
  • Post-irradiation and post-tumour scoliosis

for adults with:

  • spine deformity acquired in childhood / adolescence and where it has been demonstrated that there is no degenerative aetiology of the condition
  • severe Scheuermann’s kyphosis
  • high grade spondylolisthesis
  • late onset or missed idiopathic scoliosis
  • kyphosis due to Ankylosing Spondylitis
  • problems following previous spine deformity surgery

For adults only plain X-rays are required with AP and lateral views placed onto the PACS National Archive for review prior to reviewing the referral. Please mark adult referrals to “Adult Service”.

We are not allowed to accept patients with a degenerative scoliosis or other problem.
