REACH Heart Failure Manual

NHS Lothian is delighted to be able to offer the REACH Heart Failure Programme.

Produced in collaboration with colleagues in the Rehabilitation Enablement in Chronic Heart Failure (REACH-HF) team, the Heart Failure Manual is a facilitated, evidence-informed, self-management programme for individuals living with heart failure and their family/caregivers. The REACH HF Manual helps them manage their condition, based on the principles of cardiac rehabilitation. Funded by NIHR, both its clinical effectiveness and cost effectiveness have now been evidenced and published.

Recently, the REACH-HF team won the BMJ 2020 Stroke and Cardiovascular Team of the year award. The REACH-HF programme is listed on the NICE shared learning website and has been included in the NICE CVD Impact report.

Training & Heart Failure Manuals
Training and procurement of the REACH-HF programme is being led by clinicians in the Heart Manual Department. Topics covered in training will enable you to confidently facilitate the programme, and include areas such:

  • Self-management in Heart Failure
  • The psychological aspects of Heart Failure
  • Health behaviour change
  • Supporting family and caregivers
  • Physical activity and chair based exercise
