The CHSS Stroke Nurse Services work in partnership with the NHS in each area and complements the existing local provision of services.
Aims of the service
The aim of the service is to facilitate a smooth transition from hospital back into the community for those people who have had a stroke, by supporting them, and their carers to adjust to the changes.
Objectives
- To give advice, information, support, raise awareness of risk factors, encourage appropriate lifestyle changes, and help minimize the risk of further strokes through education. To identify and help solve problems and concerns.
- To facilitate reintegration to home and the community.
- To enable fostering of independence on the part of the patients and their carers and promote self management.
- To ensure the role of the stroke nurse is one within the multidisciplinary team.
- To work closely with other voluntary sector colleagues and social care in the wider community.
Contacting the service
There are five stroke liaison nurses and they are based in each of the stroke units across Lothian. They introduce themselves to the patients while they are still inpatients and visit them in their homes shortly after they have been discharged home.
