Referral information for DNA tests

Users are requested to print copies of these forms as required.  Should this be impossible, additional copies of forms can be obtained from;

Completion of request form

  • The completed form must have legible patient details complying with the Mandatory Data Set requirements.  Preferably stick a printed label on the form, or use a black pen to write the patient data and clinical information. 
  • Name and address of clinician(s) to whom the report is to be sent must be clearly indicated on the form accompanying the sample.
  •  Referral reason should also be provided to prevent delays in testing.

Known (family) mutations

The relationship to the individual originally tested should be supplied; preferably as a pedigree.  For mutations identified in our lab, the disease/gene along with name and DoB of the relative tested are sufficient.  If the mutation was identified elsewhere a copy of the original laboratory report should be provided.  If this is unavailable, information about the individual tested (e.g. name, DoB, address), disease/gene tested for, and the name of the testing laboratory are required.

Information needed for tests in other laboratories

When referring rare conditions for testing at other centres, clinicians should note that the receiving laboratory will often require detailed information on the patient and family.  In many cases, testing will only proceed on receipt of completed forms issued by the receiving laboratory.  It is the responsibility of the referring clinician to provide any information required by the receiving lab:  the DNA lab does not hold copies of such forms, but staff will be happy to offer advice on their completion and can provide web site links to paperwork, contact email addresses etc.

Test requests from other Genetics laboratories

We do not require completion of a standard form:  Minimum data required with sample are the same as on appropriate referral form plus referring laboratory’s sample number.  Sample tubes must be labelled with a minimum of either patient name and DoB or referring laboratory number.  Samples without minimum data, or when data on sample and accompanying documents do not match, will not be accepted.

 DNA samples from any accredited U.K. laboratory outside of the SGMC must include an “Out of Area Test” (OAT) form and/or name and address for billing.  Failure to provide this information may lead to delays in testing.
