Edinburgh Hub Services

Who we are?  What we do?

We are an Integrated City of Edinburgh Council and NHS Lothian Service specialising in providing person centred assessments and rehabilitation in your own home. 

We provide short term input, up to approximately 6 weeks, by a multi-disciplinary team of Physiotherapists, Occupational Therapists, Assistant Practitioners, Therapy Assistants, Social Workers, and Rehabilitation-led care providers. 

You may be referred to us if you have:

  • Had a sudden deterioration in your physical functioning at home and are at risk of hospital admission because of this.
  • Have recently been discharged from hospital and require Physiotherapy and Occupational Therapy guided rehabilitation to support your return to independence.
  • Have had a recent fall, or are at risk of falling.

Our teams are based in four localities across the city:

How to access?

A referral can be made by your GP or a Health / Social Care professional either in the Community or in Hospital.

How long will you wait?

  • For urgent assessments to prevent hospital admission, we aim to see you within 24-48 hours of receiving the referral.
  • For the falls prevention pathway, we aim to see you within 7-10 days.
  • For rehabilitation following hospital discharge, we aim to see you within a couple of weeks of you leaving hospital.  More accurate timings can be discussed at time of referral with the hospital professional.

Your first appointment, what to expect?

You will be contacted by our team to book a visit to your home.  Our first visit will take about an hour. 

You will receive a visit at home by a Physiotherapist and Occupational Therapist to complete a holistic, multifactorial assessment, discuss what matters to you and what you would like to aim to be able to achieve.   It would be helpful to have a think about the things that are important to you, so that we can help you to set your own personal goals.
