Referral Criteria:
The service is for adults in the Lothians living with a progressive neurological condition like Multiple Sclerosis, Huntingtons Disease, Multi-System Atrophy or Cerebellar Ataxia.*
- Living at home or in supported accommodation
- Where the condition is impacting on wellbeing, and participation in daily life.
- Where ongoing consultancy from a specialist MDT (multi-disciplinary team) would support them, and their family to live well with their condition.
- Where joint working with local primary care and community services would benefit the person and their family.
- Please ensure that the person you are referring is in agreement with the referral
[* There are separate services that may be more appropriate for people with Dementia, Motor Neurone Disease and Parkinsons Disease].

PLEASE NOTE We are needing to review our current service in light of the Covid-19 situation, but our usual service features:
- Ongoing relationship with Lanfine team- regular outreach MDT review and direct access to the team for advice and support in between planned MDT reviews
- Specialist outreach assessment with access to full MDT as required
- Short term outreach intervention and consultancy
- Planned inpatient admission (up to 2 weeks) for goal focussed assessment and rehabilitation (in only those exceptional circumstances where what’s needed cannot be achieved with support in the community). This service is limited, and we do not offer emergency admissions, transfer from acute hospital or respite admissions.
- Signposting and referral on (e.g. District Nursing Team, SMART Centre, Community OT and Physiotherapy Services)
- Joint working with local services
- Carer Support
