You can refer a child to our service by either:

calling us on 01506 524134 (you can leave a message on our answer machine with a brief message and your contact number if no answer)
or by downloading, printing and posting this form to:

Please note, we are unable to accept emails from parents/family/carers at present – any emails or referrals sent by parents/family/carers will be deleted instantly.
