(Also called GPAPP or First Contact Physiotherapy)
You might see a specialist physiotherapist at your GP practice instead of a doctor. This person is called an Advanced Practice Physiotherapist (APP). They are also known as a GPAPP or First Contact Physiotherapist.
The UK Government introduced this role in 2018 to help doctors and support people to get expert care quickly for muscle, joint, and bone problems.
APPs are trained to help with these problems. You don’t need to see a doctor first.
Service feedback:
- In 2024–2025, 97–98% of patients said they were happy with the care they got from the West Lothian APP service. This was based on a survey of 420 patients.
- Patients said the team were friendly, professional, and easy to talk to, and gave clear answers that helped them feel confident and well cared for.

What can an APP help with?
APPS Can:
- Listen to you and give advice
- Check your problem and help you feel better
- Ask for X-rays or tests if needed
- Send you to other services if needed
- Notice when more help is needed and make sure you get the right care
- Help with medicine or Fit Notes if needed
How do I book an appointment?
Just contact your GP practice reception team.
You don’t need a referral from a doctor.
How long will I wait?
There is no waiting list for this service.
Ask your reception team about available appointments.
What happens at the appointment?
- Your appointment may last up to 20 minutes.
- We’ll talk about your symptoms.
- We’ll ask questions about your health.
- We’ll usually do a physical check of the area that’s bothering you.
Please wear clothes that make it easy to see or move the area (like a vest or shorts).
What happens next?
- We’ll give advice on exercises and ways to help you get better.
- We can refer you to local services like physiotherapy services or Xcite.
- If you need more tests or help (like an X-ray or seeing a specialist), we can arrange that too.
What if I need to see a GP?
GPAPPs work closely with doctors and nurses.
If we think you need to see someone else, we’ll help arrange it.
We can:
- Refer you to a GP or nurse
- Order tests and X-rays
- Tell you what to do next and how to follow up if needed
