VOCAL and the Edinburgh Carer Support Team are discussing with unpaid carers their preference for care and treatment should they become unwell. They are also helping them to think ahead about putting alternative care arrangements in place.

VOCAL supports carers to complete an ACP form to take to discuss with their GP practice team, requesting the information is shared across the integrated system through ACP-KIS.
