Where secure email addresses exist these can be used for direct communication between the care home and NHS services for resident and staff related information.
This includes submission or referral forms for different specialist services and communication with the Health Protection and Public Health Teams.

West Lothian Care Home Liaison Team
West Lothian Care Home Liaison Team, is a relatively new team with a wide range of knowledge and experience. The team comprises of:

- Advance Nurse Practitioner – Ashley Houston
- Advanced Nurse Practitioner – Louise Kelly
- Nurse Practitioner – Aisha Akyeampong
- Nurse Practitioner – Kirstie Tidy
- Staff Nurse – Rhona Allardice
Our extended Multidisciplinary Team includes input from two Social Workers, Patricia Burns and Michael Fitzpatrick. Additionally, we have recently secured funding for a specialist bowel and bladder nurse who we hope to have in post soon.

The team provide:
- Advice and information on IPC in conjunction with Public Health Team
- Training and support on Restore mini tool, NEWS 2 and SBAR to help staff identify and support symptoms for those residents with Covid-19.
- Support care home staff with wound care, palliative care and management of Long Term Conditions
- Support the winter flu and Covid-19 vaccination programmes
- Hospital @ home level of support to residents at risk of hospital admission and support a period of transition when residents are newly admitted to a care home.
