Neonatal Community Team

We are a robust team consisting of 7 members who are neonatal trained. We provide specialist care in the home for babies who have been discharged home early from the Neonatal Unit. Our aim is to ease the transition of care from hospital to home as smooth as possible, and as early as possible to keep mum’s and babies together.

We provide nursing care, professional advice, support and training to parents and carers to enable them to be confident in caring for their baby.

We support naso-gastric tube feeding in the home, breastfeeding support, home oxygen and home phototherapy. We also do blood tests in the home to minimise the visits families have to make to the hospital unnecessarily. We work seven days a week, and we work jointly and closely with the other health professionals: Consultant Neonatologist, Physiotherapists, Speech and Language Therapists, Dieticians, Health Visitors, Midwives, Community Children’s Nursing Team and Social Services
