The 7-Steps to future care planning / ACP in Care Homes was adapted to provide resources for practitioners supporting people living at home with severe frailty. These documents have been developed for implementation by Community Nursing teams. Feedback on their use and uptake will inform future improvements as part of ongoing review of the resources.
