Who are we?
We are a Physiotherapy team within Edinburgh Health & Social Care Partnership who specialise in assessment and rehabilitation for people in their own home or homely setting.
What we do?
Physiotherapists help people affected by injury, illness or disability through movement and exercise, manual therapy, education and advice.
Physio@Home offer a physiotherapy service to any resident in a care home, whether private or local authority owned. Our physiotherapists and health care support workers, will work with patients, their families and carers to improve quality of life and achieve person centred goals.
We can offer advice and provide equipment to minimise falls risk, especially for residents living with dementia / frailty. We offer a specialised service to advise you on positioning to protect and restore body shape for those residents with postural care needs.
We will collaborate with other health and social care professionals to meet theses outcomes.
What conditions will we see?
Will see care home residents with:
- Recent fractures, joint replacements and recent hospital discharges
- Back pain
- Falls
- Reduced mobility/ walking aid assessments
- Postural care needs e.g. contractures, difficulty with positioning (please refer to the Mansfield Checklist to help you to identify these patients).
- An acute chest infection
