The CCSHTs aim is to support Health and Social Care workers to deliver safe and effective person centred care through collaboration across services to prioritise and provide support across Lothian. The team consists of a variety of teams from Care Home Support Teams (CHST) to Specialist Teams and teams under Care Home Programme Team.
At the beginning of the year we announced on Latest News that the funding for the Collaborative Care Home Support Team had become permanent and published details of the teams that were members of the CCHST alongside their contact details; this information was also emailed to all Care Homes.
As Latest News posts only last four weeks we have developed a webpage for the CCHST which can be found in the A-Z of the website. Teams vary slightly between Older Adult Care Homes and Adult Care Homes so please view the webpage to see who offers support for your type of Care Home.
