The care home dietitians are excited to share with you our newly launched referral process.
We have amended our referral form to ensure we have all relevant information to triage residents as efficiently and appropriately as possible.
In addition, we are now asking that a food first record is completed for 4 weeks and sent along with the referral form. The food first record is used to show all the first line measures implemented by the care home prior to dietetic input. It will help ensure food first measures are fully explored before seeking dietetic support and ensure dietititians can see residents most in need quickly.
Over the next month we will continue to accept referrals on the old referral form or without food first record but as of 1st July 2025 only referrals following the new process will be accepted.
