The following pages provides GP Practice Teams guidance and practical resources for creating, recording and sharing future care plans.
Until September 2023 future care planning was referred to as anticipatory care planning (ACP). Some website resources produced before this date retain the language of ACP but remain useful and relevant for those working on future care planning.
- What constitutes a high quality KIS?
- Managing KISs in General Practice (Updated July 2023)
- Managing KISs Created without consent during Covid (Updated July 2023)
- e-REDMAP: Care planning in the Community (Updated September 2023)
There are locally designed processes to involve other community health and social care teams in gathering information for a KIS that are already established in Lothian. We would encourage their use rather than the ‘Essential Future Care Planning’ document.

Further guidance for GP Practice teams are also provided including a visual on how to create a new, or, update an existing KIS using VISION or EMIS patient record systems.
