We speak to you before and after the operation to check you are personally happy. We want to fulfil our duty to you in the\u00a0<a href=\"https:\/\/www2.gov.scot\/Topics\/Health\/Policy\/Patients-Rights\" target=\"_blank\" rel=\"noreferrer noopener\">Patients Rights (Scotland) Act 2011<\/a>. There is a\u00a0<a href=\"https:\/\/www.gov.scot\/publications\/charter-patient-rights-responsibilities-2\/\" target=\"_blank\" rel=\"noreferrer noopener\">Charter of Patients Rights and Responsibilities<\/a>\u00a0to go with the most recent published 27\/06\/2019.<\/p>\n\n\n\n<p>To check all operations we do more. This fits with the Healthcare Improvement Scotland\u2019s mission of \u201c&#8230;<em>better quality health and social care for everyone in Scotland<\/em>\u201d. They advise use of standard questionnaires, called\u00a0<a href=\"http:\/\/www.healthcareimprovementscotland.org\/our_work\/person-centred_care\/proms_questionnaire_project.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Patient Reported Outcome Measures (PROMs)<\/a>. The electronic system we currently use is called the\u00a0<a href=\"https:\/\/www.britishspineregistry.com\/\" target=\"_blank\" rel=\"noreferrer noopener\">British Spine Registry (BSR)<\/a>.<\/p>\n\n\n\n<p>When things go wrong we make sure you know as we believe in being open and transparent. This is called Duty of Candour detailed in the\u00a0<a href=\"https:\/\/www2.gov.scot\/Topics\/Health\/Policy\/Duty-of-Candour\" target=\"_blank\" rel=\"noreferrer noopener\">Duty of Candour Act 2016<\/a>\u00a0and the\u00a0<a href=\"https:\/\/www2.gov.scot\/Topics\/Health\/Policy\/Duty-of-Candour\" target=\"_blank\" rel=\"noreferrer noopener\">Duty of Candour (Scotland) Regulations 2018<\/a>.<\/p>\n\n\n\n<p>In the hospital there are ways of discussing and reporting any issues when things go wrong or there are problems. We record any issues on your medical records on a system called TRAK. There are operation records on a system call ORSOS.<\/p>\n\n\n\n<p>There is an\u00a0<a href=\"https:\/\/policyonline.nhslothian.scot\/Policies\/Pages\/Adverse-Event-Management-Policy.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Adverse Event Reporting System<\/a>\u00a0called DATIX with this and other policies for NHS Lothian Hospitals on the internet from July 2019. Along with this we aim to comply with the\u00a0<a href=\"http:\/\/www.healthcareimprovementscotland.org\/our_work\/patient_safety\/scottish_mortality__morbidity.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Scottish Mortality and Morbidity Programme<\/a>\u00a0(M&amp;M). We have meetings for the whole team every 3 to 4 months. We started using the hospital TRAK M&amp;M from 16\/07\/2019 so these discussions are on your medical record.<\/p>\n\n\n\n<p>Just ask if you want to know anything else about your care and tell us how you want us to feedback to you.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>This is the most important question we are most often asked.
