If the details match what we hold on our system you will be sent a registration email from the Personal Community System within 24hours with details of how to activate your account.<\/em><\/p>\n\n\n\n<p><em>Please note that this system is only available to patients aged 18 and over.<\/em><\/p>\n\n\n\n<p><em>You may also be asked by the service (admin or clinical staff) to provide an email address, or to check the email address held is correct, either during your appointment or at the point of booking your appointment. The staff can then mark you for enrolment to Personal Community without the need for you to complete the form below.<\/em><em>If you have problems using this form, please contact our support team by
