Working in collaboration with the general practitioner and community nursing team, we:
- advise on pain and symptom assessment and management
- provide psychological, spiritual and social support for patient and family
- offer advice and information
- liaise with and support other professionals involved in the patient’s care
- assist patients and their carers in making informed decisions about treatment and future care, including best place of care
- offer short-term bereavement support
A senior CNS from each of the community teams based at Marie Curie Hospice, Edinburgh, the West Lothian Community Team at St John’s Hospital, and the community team at St Columba’s Hospice Care are available to provide specialist support for patients already known to the specialist community team – there is a dedicated phone number for this service which is provided for patients. This service operates during the day at weekend and public holidays. Patients and families are given details of this service by the CNS team.
Services offered Monday to Friday 9am to 5pm:
- telephone advice and information
- specialist assessment of patients in their own homes or care homes
