Working in collaboration with other professionals, who remain responsible for the patient’s care, we:
- advise on pain and symptom assessment and management
- provide support and information for the patient, their family and friends
- assist patients and their carers in making informed decisions about treatment and future care, including best place of care
- liaise with other specialist palliative care services in hospices, the community and other hospitals
- support and educate hospital staff

- telephone advice and information
- specialist assessment of inpatients and hospital outpatients
- regular review of patients and families with complex needs
