If no label is available the following information must be legibly written<\/p>\n\n\n\n<ul class=\"has-alert-red-color has-text-color wp-block-list\"><li>Patient Identifier Number (CHI or Hospital)&nbsp;<\/li><li>Surname, Forename, Date of Birth<\/li><li>Gender and Location.&nbsp;&nbsp;<\/li><\/ul>\n\n\n\n<p>If the Patient Identifier Number is not available, the data set&nbsp;<strong>must<\/strong>&nbsp;include the first line of the address and postcode.&nbsp;<\/p>\n\n\n\n<p>Failure to comply with this policy will result in the sample being rejected or the result delayed until the necessary information is provided.&nbsp;&nbsp;<\/p>\n\n\n\n<p><strong>Referral reason<\/strong>&nbsp;should also be provided to prevent delays in testing.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Mandatory Data Set Requirements Referring clinicians are requested to comply with NHS Lothian Policy on Mandatory Data set requirements for patient identification.&nbsp;&nbsp; Hospital labels should be used wherever possible.
