{"id":299,"date":"2022-01-31T11:41:04","date_gmt":"2022-01-31T11:41:04","guid":{"rendered":"https:\/\/services.nhslothian.scot\/ecps\/edinburgh-hub-services\/"},"modified":"2022-02-01T14:24:31","modified_gmt":"2022-02-01T14:24:31","slug":"edinburgh-hub-services","status":"publish","type":"page","link":"https:\/\/services.nhslothian.scot\/ecps\/edinburgh-hub-services\/","title":{"rendered":"Edinburgh Hub Services"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\">Who we are?&nbsp; What we do?<\/h2>\n\n\n\n<p>We are an Integrated City of Edinburgh Council and NHS Lothian Service specialising in providing person centred assessments and rehabilitation in your own home.\u00a0<\/p>\n\n\n\n<p>We provide short term input, up to approximately 6 weeks, by a multi-disciplinary team of Physiotherapists, Occupational Therapists, Assistant Practitioners, Therapy Assistants, Social Workers, and Rehabilitation-led care providers.\u00a0<\/p>\n\n\n\n<p>You may be referred to us if you have:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Had a sudden deterioration in your physical functioning at home and are at risk of hospital admission because of this.<\/li><li>Have recently been discharged from hospital and require Physiotherapy and Occupational Therapy guided rehabilitation to support your return to independence.<\/li><li>Have had a recent fall, or are at risk of falling.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Where are we based?<\/h2>\n\n\n\n<p>Our teams are based in four localities across the city:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>North West Locality Hub, Edinburgh Health and Social Care Partnership, 8 West Pilton Gardens, Edinburgh, EH4 4DP<\/li><li>North East Locality Hub, Health and Social Care Partnership, East Neighbourhood Office, 101 Niddrie Mains Road, EH16 4DS<\/li><li>South West Locality Hub, Health and Social Care Partnership, Wester Hailes Healthy Living Centre, 30 Harvester Way, Edinburgh, EH14 3JF<\/li><li>South East Locality Hub, Health and Social Care Partnership, West Wing, 40 Captains road, Edinburgh, EH17 8QF<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">How to access?<\/h2>\n\n\n\n<p>A referral can be made by your GP or a Health \/ Social Care professional either in the Community or in Hospital.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How long will you wait?<\/h2>\n\n\n\n<ul class=\"wp-block-list\"><li>For urgent assessments to prevent hospital admission, we aim to see you within 24-48 hours of receiving the referral.<\/li><li>For the falls prevention pathway, we aim to see you within 7-10 days.<\/li><li>For rehabilitation following hospital discharge, we aim to see you within a couple of weeks of you leaving hospital.&nbsp; More accurate timings can be discussed at time of referral with the hospital professional.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Your first appointment, what to expect?<\/h2>\n\n\n\n<p>You will be contacted by our team to book a visit to your home. &nbsp;Our first visit will take about an hour.&nbsp;<\/p>\n\n\n\n<p>You will receive a visit at home by a Physiotherapist and Occupational Therapist to complete a holistic, multifactorial assessment, discuss what matters to you and what you would like to aim to be able to achieve.&nbsp;&nbsp; It would be helpful to have a think about the things that are important to you, so that we can help you to set your own personal goals.&nbsp;&nbsp;<\/p>\n\n\n\n<p>For example, &#8220;It matters to me that I remain as independent as possible within my own home&#8221;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What resources are there to support my health condition?&nbsp;<\/h2>\n\n\n\n<p>In the meantime, you may find useful information in the&nbsp;<a href=\"https:\/\/services.nhslothian.scot\/ecps\/resources-to-support-your-condition\/\"><strong>Resources to Support your Condition<\/strong><\/a>&nbsp;section.<br>Below are some links that you may find of particular interest:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"http:\/\/www.edinburgh.gov.uk\/info\/20076\/adults_and_older_people\" target=\"_blank\" rel=\"noreferrer noopener\">City of Edinburgh Council &#8211; Adults &amp; Older People<\/a><\/li><li><a href=\"http:\/\/www.edinburgh.gov.uk\/info\/20199\/get_care_and_support\/51\/ask_social_care_direct_for_advice\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a><\/li><\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Who we are?&nbsp; What we do?
