This can affect your appetite and ability to consume adequate amounts of food and fluids to meet your nutritional requirements. This can cause weight loss, muscle weakness, lack of energy, mobility problems, mood changes, poor wound healing and increased risk of infections.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong><em>TYPE 2 DIABETES<\/em><\/strong> <\/h4>\n\n\n\n<p>Adults who are unable to attend DESMOND education sessions (Diabetes Education and Self Management for Ongoing and Newly Diagnosed).<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>HOW TO ACCESS COMMUNITY DIETETIC SERVICE<\/strong><\/h4>\n\n\n\n<p>Referral from your GP or other Health Care Professional to the Community Dietetic Team.
