Health and Social Care Partnerships.<\/p>\n\n\n\n<p class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">These short sessions are designed for all staff working with people living in care homes, including non &#8211; clinical support staff.<\/p>\n\n\n\n<p>The sessions can be delivered locally, across the Lothians, in care homes.<\/p>\n<\/div><\/div>\n\n\n\n<div data-scroll-offset=\"0\"><h2 id=\"at-77030\" class=\"c-accordion__title js-accordion-controller\" role=\"button\">Session 1 &#8211;  Introduction to Frailty and Falls<\/h2><div id=\"ac-77030\" class=\"c-accordion__content\">\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<p class=\"has-regular-font-size\">This is an introductory session for all staff on the subject of falls and frailty.<\/p>\n\n\n\n<p class=\"has-regular-font-size\">Learning Outcomes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-regular-font-size\">LO1: Describe the reasons people fall including an introduction to frailty<\/li>\n\n\n\n<li class=\"has-regular-font-size\">LO2: Identify falls prevention strategies<\/li>\n\n\n\n<li class=\"has-regular-font-size\">LO3: Use a falls flow chart to explain how to manage a fall<\/li>\n\n\n\n<li class=\"has-regular-font-size\">LO4: Demonstrate what to do post falls regarding who to notify and resident coming home from hospital<\/li>\n<\/ul>\n\n\n\n<p>For more &#8211; Falls<\/h2><div id=\"ac-77031\" class=\"c-accordion__content\">\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<p>This session follows on from Session 1, it is for staff who are involved in completing paperwork related to falls such as risk assessment, handovers and incident reporting.<\/p>\n\n\n\n<p>Learning Outcomes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">LO1: Demonstrate an understanding of risk factor assessment and management by using a multifactorial risk assessment tool<\/li>\n\n\n\n<li class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">LO2: Demonstrate the use of SBAR-D to handover residents and when completing paperwork<\/li>\n\n\n\n<li class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">LO3: Demonstrate the use of falls cross, incident report and falls data spreadsheet<\/li>\n<\/ul>\n\n\n\n<p>For more wp-block-group-is-layout-constrained\">\n<p style=\"font-style:normal;font-weight:400\">An optional comprehensive frailty session which can be added to Session 1 with an additional 45minutes added. <\/p>\n\n\n\n<p style=\"font-style:normal;font-weight:400\">Or it can be facilitated as a 1 hour stand alone session.<\/p>\n\n\n\n<p style=\"font-style:normal;font-weight:400\">Learning Outcomes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li style=\"font-style:normal;font-weight:400\">LO1: Describe frailty and frailty syndromes<\/li>\n\n\n\n<li style=\"font-style:normal;font-weight:400\">LO2: Demonstrate an understanding of the Clinical Frailty Scale and be able to calculate a frailty score<\/li>\n\n\n\n<li style=\"font-style:normal;font-weight:400\">LO3: Demonstrate an understanding of the goals of frailty care<\/li>\n\n\n\n<li style=\"font-style:normal;font-weight:400\">LO4: Describe management strategies for frailty and frailty syndromes<\/li>\n\n\n\n<li style=\"font-style:normal;font-weight:400\">LO5: Discuss the risks associated with frailty and the management strategies for
