The Care Home Programme Team was established in May 2020 and works collaboratively with Adult Care Homes and related teams across Lothian. This includes working with Health and Social Care Partnerships support teams and services  to establish a means for escalation and oversight.

Our team consists of the Lothian Care Academy (LCA)<\/a>, OSCaRS & Palliative Care<\/a>, and the Quality Improvement Team for Care Homes and Primary Care Settings<\/a>.

Our work is linked to the recommendations within My Health, My Care, My Home Healthcare Framework for Adults Living in Care Homes<\/a> and the Health and Social Care Standards<\/a>.<\/p>\n\n\n\n
