New diabetic foot ulcerations should be referred to the community podiatrist, community podiatrist will liaise with acute diabetes foot clinic and arrange onward referral where appropriate. &nbsp;<\/li>\n\n\n\n<li>A person who has fallen due to a foot\/ankle problem<br>People with diabetes who have a foot condition who are at risk of ulceration.<\/li>\n<\/ul>\n<\/div><\/div>\n\n\n\n<p class=\"has-light-accent-grey-background-color has-background has-regular-font-size\" style=\"font-style:normal;font-weight:400\">Diabetes foot risk assessments can be carried out by any trained healthcare worker. For guidance on who to refer please use the&nbsp;&nbsp;<a href=\"https:\/\/services.nhslothian.scot\/carehomes\/wp-content\/uploads\/sites\/31\/2022\/09\/Traffic-Lights-October-2021.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Diabetes Foot Risk Stratification Triage Tool<\/a><\/p>\n\n\n\n<div class=\"wp-block-group has-light-accent-plum-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<p class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\"><strong>Requests will not be accepted for routine nail cutting, fungal nails (including taking&nbsp;clippings)&nbsp;skin care (including corns and callous) in the absence of one or more of the following medical categories:&nbsp;<\/strong><\/p>\n\n\n\n<ul style=\"font-style:normal;font-weight:400\" class=\"has-regular-font-size wp-block-list\">\n<li>Diabetes (Moderate or high foot risk status)<\/li>\n\n\n\n<li>Peripheral arterial disease<\/li>\n\n\n\n<li>Neuropathy<\/li>\n\n\n\n<li>Rheumatoid Arthritis<\/li>\n\n\n\n<li>Neurological disorders (For example, Multiple Sclerosis, Parkinson&#8217;s Disease, Stroke)<\/li>\n\n\n\n<li>Undergoing chemotherapy<\/li>\n\n\n\n<li>Non- traumatic foot\/ leg amputation<\/li>\n\n\n\n<li>Renal Problems (Chronic Kidney Disease stage 4 or 5)<\/li>\n\n\n\n<li>Steroid Therapy (oral)<\/li>\n\n\n\n<li>Immuno-suppressed<\/li>\n<\/ul>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h2 class=\"wp-block-heading\">How do I refer?<\/h2>\n\n\n\n<p class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">The Podiatry Department triage all new patient referrals on a daily basis. Please provide us with as much information as possible and include all relevant medical history and medication. Priority appointments are determined based on the information provided within the referral.<\/p>\n\n\n\n<ul style=\"font-style:normal;font-weight:400\" class=\"has-light-accent-green-background-color has-background has-regular-font-size wp-block-list\">\n<li>Care home staff can refer direct and do not require the GP to make the referral. Most care home residents will require a home visit. Some of the referrals might be resolved by a telephone consultation, especially if it&#8217;s mainly advice. Some others will require a video consultation (Near Me) or a Face to Face appointment.<\/p>\n\n\n\n<p>The patient may already be on our caseload. In those cases (or if the patient was seen in the last 6 months by our department) a phone call would be enough to let us know of any new problems\/complications and a podiatrist of the team would contact to request further information.
