Who are we?<\/h2>\n\n\n\n

We are a Physiotherapy team within Edinburgh Health & Social Care Partnership<\/a> who specialise in assessment and rehabilitation for people in their own home or homely setting.   <\/p>\n\n\n\n

How do you refer?<\/h2>\n\n\n\n

It is good practice to have a conversation with the resident \/ their relative and a senior member of the care home team prior to making a referral.<\/p>\n\n\n\n

If your resident has recently been in hospital, the hospital team may have already made a referral. <\/p>\n\n\n\n

If your resident has a chest infection, please speak to their GP, who must make the referral for physiotherapy.  <\/p>\n\n\n\n

Incomplete referral forms will be returned to the referrer.<\/strong><\/p>\n\n\n\n

What patients do we see urgently?<\/h2>\n\n\n\n

We will see some patients more urgently than others.  Please include as much information as you can on the referral form to allow us to triage accordingly. Examples of patients we see urgently may include residents who have had an acute chest infection, recent fractures, recent hospital admission where the resident still has rehabilitation needs and those with postural care needs. <\/p>\n<\/div><\/div>\n\n\n\n

Expected waiting times?<\/h2>\n\n\n\n

In the event of an acute\/ urgent referral we will endeavour to see patients within 6 weeks subject to external constraints.  Routine patients will be contacted within 12 weeks.<\/p>\n<\/div><\/div>\n\n\n\n

Does a resident require a review of their walking aid?<\/h2>\n\n\n\n

Please complete the self-referral for walking aids<\/a> referral form and a healthcare support worker will be in touch to discuss replacing your walking aid.<\/p>\n<\/div><\/div>\n\n\n\n
