{"id":547,"date":"2022-02-07T23:18:04","date_gmt":"2022-02-07T23:18:04","guid":{"rendered":"https:\/\/services.nhslothian.scot\/carehomes\/referral-pathways\/referral-pathways-pharmacy\/"},"modified":"2023-02-08T10:57:51","modified_gmt":"2023-02-08T10:57:51","slug":"referral-pathways-pharmacy","status":"publish","type":"page","link":"https:\/\/services.nhslothian.scot\/carehomes\/referral-pathways-pharmacy\/","title":{"rendered":"Pharmacy: Referral Pathway"},"content":{"rendered":"\n<div class=\"wp-block-group has-light-accent-green-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h2 class=\"wp-block-heading has-text-align-center\">Too many Care Home residents are<br>not getting the best from their medicines <br>or are taking medicines which are <br>doing them more harm than good and may be reducing their quality of life.<\/h2>\n\n\n\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"wp-block-heading\">Referral process for a medication review for a Care Home Resident<\/h3>\n\n\n\n<p class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">Primary care pharmacists, working in GP practices, are well placed to carry out a comprehensive medication review, if required, for residents.&nbsp; Pharmacists are experts in medicines, and working with the GP and the practice-based team they can ensure Care home residents receive the right medicines at the right time and in the right way to maximise the benefits from their medication.<\/p>\n\n\n\n<p>Primary care pharmacists also have links with Medicine of the Elderly (MOE) consultants in secondary care and can seek expert opinion if required. &nbsp;The pharmacy team also have allocated sessions with MOE specialists specifically for collaborative working\/reviewing medicines in this patient group.<\/p>\n\n\n\n<p>It is important, where possible, that the resident (and\/or their family members or carers, as appropriate, and in line with the resident&#8217;s wishes) and care home staff are involved in the medication review. The pharmacist, where possible, will explain the purpose of the medication review to the resident (and\/or their family members or carers) and establish what their views and understanding of the medicines prescribed are.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"wp-block-heading\">Who should I refer?<\/h3>\n\n\n\n<p class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">The Royal Pharmaceutical Society believes&nbsp;<em>&#8220;all residents should receive a review of their medication by a pharmacist when they first move into a care home in order to optimise their medication regime&#8221;<\/em>&nbsp;(see&nbsp;<a href=\"https:\/\/www.rpharms.com\/Portals\/0\/RPS%20document%20library\/Open%20access\/Policy%20statements\/improving-medicines-use-for-care-home-residents-(wales).pdf?ver=2016-10-13-162139-760\" target=\"_blank\" rel=\"noreferrer noopener\">Improving Medicines Use for Care Home Residents<\/a>&nbsp;(March 2016).&nbsp;<em>&#8220;Every care home resident should have a pharmacist led medicines review at least once a year or whenever a medicine is started, stopped or changed and when a resident moves between care settings&#8221;<\/em>&nbsp;(see&nbsp;<a href=\"https:\/\/www.rpharms.com\/resources\/reports\/the-right-medicine-improving-care-in-care-homes\" target=\"_blank\" rel=\"noreferrer noopener\">The Right Medicine: Improving Care in Care Homes<\/a>&nbsp;(February 2016).<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"wp-block-heading\">Referral for a medication review is recommended for residents who are:<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>potentially on inappropriate medicines (polypharmacy).<\/li>\n\n\n\n<li>refusing to take their medicines or are &#8220;fed up taking tablets&#8221;<\/li>\n\n\n\n<li>finding it difficult to swallow their medicines<\/li>\n\n\n\n<li>sleeping a lot during the day\/agitated during the day<\/li>\n\n\n\n<li>dizzy when they stand up<\/li>\n\n\n\n<li>at risk of falls<\/li>\n\n\n\n<li>becoming increasingly frail<\/li>\n\n\n\n<li>losing weight<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-left\"><strong>or<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>frequency or timing&nbsp; of administration of medication is causing problems<\/li>\n\n\n\n<li>concerned the resident may be experiencing side effects of their medication<\/li>\n\n\n\n<li>more specific medication &nbsp;information is required about some of the medication a resident is on<\/li>\n\n\n\n<li>medication changes are unclear post discharge from hospital<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center has-light-accent-plum-background-color has-background has-regular-font-size\" style=\"font-style:normal;font-weight:400\"><strong>Please note this referral process is not suitable for any residents who become acutely unwell. In this instance seek medical\/nursing support in the usual way.<\/strong><br><br><strong>A medication review may be appropriate after the acute episode has been resolved<\/strong><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"wp-block-heading\">How can I make a referral?<\/h3>\n\n\n\n<p class=\"has-regular-font-size\" style=\"font-style:normal;font-weight:400\">To refer a resident for a medication review please email the Primary Care Pharmacy team using one of the two methods below.<br><br>Email: <a href=\"mailto:PC.Prescribing@nhslothian.scot.nhs.uk\">PC.Prescribing@nhslothian.scot.nhs.uk<\/a>.
